小儿异位型输尿管囊肿合并重复肾、输尿管畸形

目的探讨小儿异位型输尿管囊肿合并重复肾、输尿管畸形的诊断及治疗。方法对1996￣2005年收治的18例异位型输尿管囊肿合并重复肾、输尿管畸形患儿的诊治资料进行总结。结果B超诊断符合率为80%,B超结合IVU、CT诊断符合率达100%,17例患儿经手术治疗1次治愈,1例行2期手术。结论B超、IVU应为基本的检查手段,排尿时膀胱尿道造影可用于评价囊肿对肾脏的影响,上肾段及输尿管切除、囊肿切除 输尿管再植,手术方法简单,效果良好,是基本的治疗手段。根据肾脏受影响的情况选择治疗方案是一种简便、可行的选择标准。

Ectopic ureterocele associated to duplex system in children

Objective] To study the diagnosis and treatment of the ectopic ureterocele associated to duplex system in children. Methods] 18 cases of ectopic ureterocele associated to duplex system in children diagnosed at our institution between 1996~2005 were retrospectively reviewed. Initial surgical procedure and subsequent outcomes were analyzed. Results] The accurate diagnosis rate of ultrasonography has been 80% and became 100% when combinedly used with IVU and CT. Upper pole nephroureterectomy was undertaken for 4 cases, excision of ureterocele with reimplantation of the ureter for 14 cases. Surgical inter vention has been successful in 17 out of 18, further surgical measures being not needed. Only 1 case after excision of ureterocele with reimplantation of the ureter further upper pole nephroureterectomy was undertaken because of the recurrent upper pole infection. Conclusion] Ultrasonography combinedly used with IVU is an important measure in the diagnosis of ureterocele. Voiding cystourethrogram (VCUG) can be used to detect veiscoureteral reflux (VUR). Upper pole nephroureterectomy or excision of ureterocele with reimplantation of the ureter is one of the major methods for treatment. The classification of ectopic ureterocele based on renal unit in jeopardy is helpful in selecting the initial surgical strategy.
