温针灸联合牵引及红外线照射治疗腰椎间盘突出症随机平行对照研究

目的]观察温针灸联合牵引及红外线照射治疗腰椎间盘突出症疗效。方法]使用随机平行对照方法,将150例门诊及住院患者按病志号抽签简单随机分为两组。对照组75例①牵引:腰椎自动牵引床牵引,30min/d;②红外线照射:牵引完毕后进行,根据患者皮肤感觉调整距离,30min/d。治疗组75例温针灸:腰部夹脊穴、腰阳关、肾俞、阿是穴,症状循太阳经者配委中、承山、秩边、昆仑,症状循少阳经者配阳陵泉、环跳、风市、足临泣;以2寸或1.5寸毫针常规消毒后针刺,行提插泻法,得气后留针,以约2cm艾条套于针柄,点燃后根据皮温及患者感受调整距离,2cm艾条燃烧所需时间记为1壮,连续行3壮后起针,每壮之间行针1次;牵引及红外线照射治疗同对照组。连续治疗10d为1疗程。观测临床症状、腰痛评分、不良反应。治疗1疗程,判定疗效。结果]治疗组临床痊愈17例,显效31例,有效24例,无效3例,总有效率96.00%。对照组临床痊愈12例,显效26例,有效21例,无效16例,总有效率78.67%。治疗组疗效优于对照组(P0.01)。腰痛评分两组均有改善(P0.01),治疗组改善优于对照组(P0.01)。结论]温针灸联合牵引及红外线照射在治疗腰椎间盘突出症中效果显著,值得推广。
