透骨散熏洗与穴位贴敷联合基础治疗护理糖尿病周围神经病变随机平行对照研究

目的]观察透骨散熏洗与穴位贴敷联合基础治疗护理糖尿病周围神经病变疗效。方法]使用随机平行对照方法,将60例住院患者按病志号抽签简单随机分为两组。对照组30例常规护理,糖尿病基础治疗(控制血糖、糖尿病教育、饮食控制、适量运动、降糖药治疗、心理调整);温水泡脚。护理组30例透骨散熏洗(朱金山创制,透骨草、伸筋草、桑枝、刘寄奴、桂枝、赤芍、丹皮、艾叶各10g),加水浸泡10 min,水煎2000mL,晾至50~70℃,沐足桶中放踏脚凳,用热气熏洗双下肢,水温降至38~42℃时撤除踏脚凳,纱布蘸药液淋洗浸泡双足,淋洗时淋至足三里穴,浸泡时水面没过三阴交穴,每次熏洗15~20mim;熏洗完毕后穴位贴敷(朱金山创制,透骨草、伸筋草、桑枝、刘寄奴、桂枝、赤芍、丹皮、艾叶各10g),打粉加赋形剂,搅拌后摊成3×3cm,厚3mm药膏,单层绵纸包裹成药包,置足三里、三阴交,透气敷料固定,每次4~6h;常规护理,糖尿病基础治疗同对照组。连续治疗7d为1疗程。观测临床症状、不良反应。连续护理2疗程,判定疗效。结果]治疗组显效1例,有效27例,无效1例,总有效率96.66%。对照组显效1例,有效17例,无效12例,总有效率60.00%。治疗组疗效优于对照组(P0.01)。结论]透骨散熏洗与穴位贴敷联合基础治疗护理糖尿病周围神经病变效果显著,值得推广。

Traditional Chinese Medicine Fumigation Joint Point Attach Care Diabetes Peripheral Neuropathy Random Parallel Control Study

Objective]To observe the traditional Chinese medicine fumigation combined acupuncture point attach nursing diabetic peripheral neuropathy effect. Methods]Using random parallel control method,60 cases of hospitalized patients into two groups according to random number table method. The control group 30 cases of routine nursing. Strict glucose control,diabetes foundation treatment（diabetes education,diet,exercise,diabetes treatment,psychological adjustment）. Warm water to soak the foot. Treatment group 30 cases of herbal fumigation and acupoint sticking. Traditional Chinese medicine fumigation（Tougucao,Shenjincao,Sangzhi,Liujinu,Guizhi,Chishao,Danpi,Aiye10g）,add water soak 10 min,Fried 2000 ml water. Air to 50 ~ 70 ℃,foot barrels into the foot stool,use steam fumigation double lower limbs,when the temperature dropped to 38 ~ 42℃ removal foot stool,gauze dipped in leaching solution soak feet,injector during leaching to st 36 hole,soaked in water before three vaginal intercourse,fumigation 15 ~ 20 mim at a time. After Chinese medicine fumigation acupoint sticking（Tougucao,Shenjincao,Sangzhi,Liujinu,Guizhi,Chishao,Danpi,Aiye10 g）,powder and excipient,stir out of 3×3cm,3 mm thick ointment,single-layer cotton paper parcel medicine package,three vaginal intercourse,and rear foot three mile,breathable dressing is fixed,every 4 ~ 6 h. Routine nursing with the control group. Continuous treatment of 7 d for 1 course of treatment. Observation of clinical symptoms and adverse reactions. Continuous nursing 2 period of treatment,judge curative effect. Result] the treatment group 1 case had marked effect,effectively and 27 cases,1 had no effect,total effective rate was 96.66%. The control group 1 case had marked effect,17 cases effective,12 had no effect,the total effective rate was 60.00%. The treatment group curative effect is better than that of control group（P 0.01）.
