182例肾损伤诊治分析

摘    要:目的:探讨肾损伤诊断及治疗方法的选择。方法:对182例肾损伤病人的检查结果及不同治疗方法进行分析。结果:血尿152例(83.5%);B超检查66例,有异常改变43例(65.2%);CT检查86例,65例(76.1%)表现异常。136例非手术治疗,全部治愈。手术治疗29例.26例痊愈,3例死于多发伤失血性休克。17例肾动脉栓塞获得满意治疗效果。本组肾损伤治愈率98.9%.结论:血尿是诊断肾损伤的重要依据。对严重多发伤的病例,不论有无血尿.应常规进行B超和CT检查。轻中度肾损伤能用非手术疗法治愈,肾动脉栓塞对中度肾损伤疗效确切,对严重肾损伤应及时手术治疗。

Abstract:Objective: To explore the diagnostic and therapeutic methods of renal injury. Methods: One hundred and eighty six rases of renal injury were retrospectively studied. Results: Hematuria occurred in 152 of 182 (83. 5%) eases. The B-ultrasonography detected 43 out of 66(65. 2%) renal injury abnormal. 65 of 86(76. 1%) cases on CT scan were abnormal. 136 cases were managed with non-surgical treatment and all of fhem were cured. 29 cases were treated by open operation. 26 cases we're cured. 3 case died of shock and coexistant injuries of other organs. 17 cases were treated with selected renal artcriographic embolish. Therapeutic effect was satisfied. The cure rate of renal injury is 98. 9% in these cases. Conclusion: Post traumatic hematuria is one of the important symptoms for diagnosis of renal trauma. The B-ultrasonography and CT scan were highly aecurate in diagnosis of renal trauma without hematuria in multiple trauma patients. The mild to middle degree renal injury could be cured by conservative treatment. The therapeutic effect of selected renal artcrigraphic embolism for middle degree patients was quite well. Operations should be undertaken in time in cases of severe renal injury.
