摘    要:为研究急性心肌缺血对心室传导的作用,以PTCA(球囊扩张)作为急性早期心肌缺血的模型,在13例未经选择的PTCA病人分别于球囊扩张术前、术中和术后记录信号叠加心电图。结果表明,急性严重心肌缺血可引起心室传导改变,而且这种改变可随心肌缺血缓解而逆转。提示其基础是功能性而非解剖学的,解剖学改变通常发生于陈旧心肌梗塞;同时间接证实,急性心肌缺血时室性必律失常的机理是折返。

Abstract:To investigate the effects of acute myocardial ischemia on ventricular conduction, signal - averagedelectrogram were recorded in each of the 13 unselected PTCA patients before, durting and after balloon dilatation,using PTCA as a model of acute early myocardial ischamia (AEMI). The results showed that acute severe myocardial ischemia could cause ventricular conduction changes (VCC), which were reversible with the resolution of ischemia. It suggested that there was a functional basis for the development of VCC, instead of anatomical alterations,which usually occurred in old myocardial infarction. It also indicated indirectly that the mechanism of ventriculartachycardia in AEMI was reentry.
