缺血性心肌病血清内皮素、一氧化氮的变化及临床意义

摘    要:目的:探讨冠状动脉粥样硬化性心脏病(CHD)引起的缺血性心肌病患者血浆中内皮素(ET)、一氧化氮(NO)变化的临床意义。方法:测定80例CHD血浆中的ET、NO的含量,观察其中29例缺血性心肌病患者血浆中ET、NO的变化,并与20例正常健康人做对照。结果:缺血性心肌病组平均ET、NO较冠心病组ET、NO增高,较正常对照组ET、NO显著增高(P〈0.01)。用直线相关分析ET与NO之间改变的关系,ET与NO呈正相关,相关系数无统计意义。缺血性心肌病组心肌肥厚以ET增高为主,与冠心病组比较有显著性差异(P〈0.05)。扩张型心肌病组以NO增高为主与冠心病组比较有有极显著差异(P〈0.01)。结论:缺血性心肌病ET与NO增高,在促进心肌细胞增生、心肌细胞肥大的病理生理中起主要作用。NO参与扩张心肌病心肌细胞增生与重构

Abstract:Objective:To explore the changes and clinical significance of serum endothelin and nitric oxide levels in CHD patients with ischemic cardiomyopathy.Methods:The serum levels of ET and NO were measured in 80 coronary heat disease (CHD) cases,29 ischemic cardiomyopathy cases and 20 cases of the control. Results:The serum levels of ET and NO in patients with ischemic cardiomyopathy were much higher than those in CHD patients and control cases ( P <0.01). Positive correlation was observed between the ET and NO levels. ET increase was more significant in patients with ischemic cardiomyopathy and difference was observed when compared with the ET level in CHD patients ( P <0.05). NO increase was observed mainly in dilative cardiomyopathy cases and difference was also observed when compared with the NO level in CHD cases ( P <0.01). Conclusions:The increased serum ET and NO levels played an important role in the proliferation and pathophysiology in patients with ischemic cardiomyopathy and NO level was closely related to the pathophysiology in patients with dilative cardiomyopathy.
