火器伤骨折固定方法对其感染和细菌学特点的影响

目的：探讨火器伤骨折的细菌学特点和不同治疗方法对火器伤骨折感染的影响。方法：建立犬股骨火器伤骨折模型，采用石膏、钢板和外固定器等骨折固定方法，观察其细菌学特点（分类和定量），分析其感染形成的相关因素。结果：外固定器治疗组伤道细菌量较少，感染率较低。石膏固定组感染的病原菌常与伤道初期污染菌有关。外固定器和钢板固定组常见医原性感染。结论：外固定器技术能可靠固定骨折，又避免了对骨折局部的不良干扰，利于火器伤骨折感染的防治。

Effect of Vary Fixation on the Bacteriological Feature and Infection in Gunshot Fracture

Objective: To research the bacteriological feature and the effect of vary fixation on infection in gunshot fracture. Method: After canine femur were shot by gun, the fracture were fixed by plaster cast, plate and external fixator. The character of bacteriology and the factors related with infection were analyzed. Results: The quantity of bacteria was fewer, and the rate of infection was lower in the group of external fixator. Iatrogenic infection was more common in the groups of external fixator and plaster fixation groups. In the later, the infection was related to the bacteria comminuted following injury. Conclusion: The treatment method plays an important role in the infection of gunshot fracture. The technique of external fixator can achieve stable of fracture while not disturb local tissues injured, which is an excellent method to treat gunshot fracture.
