老年肺癌患者的多原发恶性肿瘤临床分析

目的 总结21例老年肺癌患者的多原发恶性肿瘤的临床诊断、治疗和预后。方法 回顾性分析老年肺癌患者多原发恶性肿瘤21例的临床资料，男17例，女4例。多原发肺癌2例，占9．5％。肺癌与其他脏器的恶性肿瘤19例，占90．5％，其中16例为双原发恶性肿瘤，占84．2％，3例为三原发性恶性肿瘤，占15．8％。结果 第一原发癌平均生存期147．14月，第一原发癌与第二原发癌间隔平均138月，第一原发癌中行手术治疗的18例患者平均生存期167．7月，3例非手术患者平均生存期11．67月，手术治疗疗效明显高于非手术治疗。结论 随着医学的发展，医患对多原发癌认识的不断增强，其早期诊断及检出率的不断提高，并采取积极的手术治疗，将会使多原发癌患者获得更加理想的疗效。

A Clinical Analysis on Multiple Primavy Malignant Tumors in Elderly Patiants with Lung Cancer

Objective To summarize the diagnosis,treatment and prognosis of multiple primary malignant tumors in 21 elderly patients with lung cancer.Methods Retrospective analysis was given to the clinical data of the 21 elderly patients,17 males and 4 females.Except 2 cases(9.5%)of multiple primary lung cancer,the left 19 cases(90.5%)were found to have lung cancer or the malignant tumors in other organs;out of the 19 cases,double primary malignant tumors were found in 16 cases (84.2%),and triple primary malignant tumors in 3 cases(15.8%).Results The mean survival of the first primary cancer was 147.14 months;the mean survival between primary tumor and the second tumor was 138 months;18 patients with the first primary cancer received operation and their mean survival reached 167.7 months,much longer than that of the 3 cases without operation,only 11.67 months.The curative effect of surgical treatment is obviously superior to the non-surgical treatment.Conclusion With the development of medicine,both doctors and patients would have more knowledge about the multiple primary cancer,and the rate of early diagnosis would be raised,which would make it possible to give active surgical treatment,and better curative effect would be gained.
