CT对腹部空腔脏器闭合性损伤的诊断价值

目的探讨CT对腹部空腔脏器闭合性损伤的诊断价值。方法对16例腹部空腔脏器闭合性损伤患者进行分析。结果本组腹部空腔脏器闭合性损伤患者，胃损伤3例，十二指肠损伤2例，空肠损伤3例，回肠损伤5例，结肠损伤3例。5例腹腔内可见气体，1例呈点状类圆形气体局限于肠系膜间伴肠系膜血肿，1例呈窄奈状气体局限于左肾旁前间隙内。腹腔积液或腹膜后积液11例。肠壁增厚或伴肠系膜血肿4例。口服可溶性造影剂外溢1例，局部胃壁周围脂肪间隙密度增高。结论对腹腔空腔脏器的损伤CT能检出X线或B超无阳性发现者，对此病早期诊断及预后具有重要价值。

The significance of spiral CT scanning in diagnosis of closed injury of abdominal viscera with lumen

Objective To explore the clinical significance of spiral CT scanning in diagnosis of closed injury in abdominal viscera with lumen. Methods The clinical data of 16 patients with closed injury of abdominal viscera with lumen confirmed by operation were re- viewed and analyzed. Results Among these 16 patients, 3 patients suffered from stomach injury, 2 from duodenal injury, 3 from jejunal injury, 5 from ileal injury and 3 from colonic injury. There was free gas in the peritoneal cavity seen in 5 cases, 1 with ring - shaped gas spots localized between mesenteries assceiated with mesenteric hematoma, 1 with narrow stripe gas localized in anterior space beside the left kidney. Free fluid in peritoneal cavity or retroperitoneum was seen in 11 cases. The thickening of intestinal wall or associated with mesenterie hematoma were seen in 4 cases. Extravasation of soluble contrast medium after oral administration and enhanced density of fat space around stomach wall were seen in 1 case. Conclusion The injury of abdominal viscera with lumen can be detected by spiral CT scanning while it was negative in X - ray examination or B - mode ultrasonography. Therefore spiral CT scanning is important in early diagnosis and prognosis for treatment of injury in abdominal viscera with lumen.
