封闭式负压引流预防腹部手术切口感染

【目的】探讨封闭式负压引流对腹部污染性或感染性手术后切口感染的预防效果及临床应用价值。【方法】取55例腹部污染性或感染性手术，手术结束前，将多孔引流管或蜂窝式引流管置于切口深处，间断缝合皮下组织及切口，引流管接50-60kPa持续负压吸引。同时采用传统方法引流80例同类患者手术切口，比较负压组与传统组的感染情况。【结果】负压组切口感染及脂肪液化率（10．9％）明显低于对照组（22．5％），在切121拆线时间、换药次数等方面，负压组与传统组比较，均有统计学意义（P〈0．05）。【结论】封闭式负压吸引能有效引流创面渗出、消除感染、刺激创面肉芽组织快速生长，是预防切口感染的有效方法。

Research on prevention cuts from bacterical infection in abdomen surgery by vacuum assisted closure

[Objective] To investigate the clinical effect of vacuum assisted closure in prevention cuts from infection in contaminated abdomen surgery. [Methods] 55 patients＇ abdomen cuts were treated during contaminated abdomen surgery by vacuum assisted closure. The wound surfaces or cavities were filled with self-designed sponge-drainage tubes and then sewed up with thread. The drainage tubes were connected with self-designed vacuum bottle （negative pressure of 50 - 60 kPa） . Besides, 80 patients with abdomen infections diseases were operated , whose cuts were managed by tradi- tional dressing as the control group. [Results] The rate of cuts infection or fat liquefaction necrosis was 22.5%in the control group, which was significantly decreased to 10.9% in experimental group. There were statistical differences in times of dressing and wound healing days between two groups （P 〈 0.05）. [Conclusions] Vacuum assisted closure can protect the wound against contamination, evacuate the wound exudates completely, stimulate the growth of granulation tissue, and facilitate the wound healing; so the vacuum assisted closure is an effective method in prevention cuts from in- fection in contaminated abdomen surgery.
