补阳还五汤佐治慢性阻塞性肺病合并肺性脑病36例临床分析

目的：探讨补阳还五汤佐治慢性阻塞性肺病（COPD）肺性脑病的临床疗效。方法：对72例确诊为COPD合并肺性脑病的患者随机分为2组,治疗组在常规治疗的基础上给予口服或胃管鼻饲补阳还五汤,观察治疗后患者意识及血气指标变化,对照组给予吸氧、抗感染、解痉平喘、排痰、无创机械通气等常规治疗。结果：治疗组有效率91.67%,明显高于对照组的63.89%（P〈0.05）,而且治疗组提高PaO2和降低PaCO2的效果也明显高于对照组（P〈0.05）。结论：补阳还五汤能明显改善COPO合并肺性脑病患者的临床症状和体征,改善酸碱失衡,提高PaO2,从而降低PaCO2,提高抢救成功率。

Chronic Obstructive Pulmonary Disease Complicated with Pulmonary Encephalopathy Treated with BuYang HuanWuTang as Adjuvant: A Report of 36 Cases

Objective： To explore curative effects of BuYang HuanWuTang in treating chronic obstructive pulmonary disease（COPD） complicated with pulmonary encephalopathy as adjuvant.Method： Seventy-two patients with proved pulmonary encephalopathy were randomly divided into two groups.Treatment group took BuYang HuanWuTang orally or through nasogastric feeding based on routine treatment,changes of consciousness and blood gas analysis were inspected.Control group were administered with oxygen inhalation,anti-inflammation,relieving spasm and asthma,dispelling phlegm,nontraumatic mechanical ventilation,and so on.Result： Effective rate of treatment group was 91.67%,clearly higher than 63.89% of control group（P0.05）,meanwhile the effects of lifting PaO2 and lowering PaCO2 of treatment group were higher than that of control group（P0.05）.Conclusion： The results showed that BuYang HuanWuTang can evidently improve symptoms and signs,improve acid-base imbalance,increase PaO2 to reduce PaCO2 and raise rate of success.
