尺骨截骨延长治疗陈旧性骨折的临床观察

目的探讨用尺骨截骨延长治疗陈旧性孟氏骨折的临床疗效。方法2001～2005年收治陈旧性孟氏骨折患者36例采用尺骨截骨延长结合挠骨头与肪骨小头吻合复位后克氏针内固定术治疗,均不行环状韧带重建。结果30例患者获1～4年平均2年随访。根据Mackay等功能评定标准本组优22例,良4例,差4例,优良率：86.7%。结论对于陈旧性孟氏骨折而言,术中通过尺骨截骨延长纠正尺骨短缩畸形是术后关键,同时术后4～5周拨出胧挠关节固定克氏针,并进行早期康复训练可以取得满意疗效。

The Osteotomy Ulna Lengthening to Treat Old Monteggia Fractures

Objective To avaluate the clinical results of the oteotomy of ulna lengthening to treat old Monteggia fractures.Methods 36 cases of old Monteggia fractures is not recommented to reconstruction of anular ligaments,both the osteotomy ulna were fixed with plate and radiohum eraljoint were fixed with Kirechnerwire which two group were treated with plaster splint after operation from 2001 to 2005.Results 30 patient were follow up 1 year to 4 years（1 years on average）.In the group 22 cases were rated as excellent, 4 good and 4 poor,the effective rate was 86.7%.The criterion of function was evaluated according to the function driteria for Mackay.Conclusion The careful treament of fracture of ulna and firm internal fixation arethe keys of treament of Monteggia fractures, and to pull off the Kirschnerwire with fixation of radiohum eral joint after operation 4～5 weeks.The early function rehabilitation is bases to protection of effect.
