外周血白细胞和血浆中EB病毒DNA含量检测的比较

摘    要:目的 探讨外周血白细胞和血浆中EB病毒DNA含量是否存在差异, 为实验室标本类型的选择提供依据。方法 收集延安大学附属医院423例疑似EB病毒感染的住院及门诊患者的抗凝全血标本, 用荧光定量PCR法, 分别于白细胞和血浆中提取EB病毒DNA, 进行PCR扩增定量检测, 并对结果进行比较分析。结果 423例标本中, 外周血白细胞标本EB病毒DNA阳性率为32.38%(137/423), 血浆标本EB病毒DNA阳性率为2.36%(10/423), 差异有统计学意义(P<0.05)。且同为阳性的标本中, 白细胞中EB病毒DNA含量均明显高于血浆, 当白细胞中EB病毒的含量达到5次方以上时, 血浆中EB病毒的含量才能达到阳性。结论 外周血白细胞和血浆中EB病毒核酸载量, 二者比较有统计学差异, 外周血白细胞检测的阳性率高于血浆, 白细胞中EB病毒的含量达到一定程度, 血浆标本中才可检测到, 以外周血白细胞为检测标本更为灵敏, 可以减少漏诊。

Abstract:Objective To investigate whether there are differences in the content of EB virus DNA in peripheral blood leucocytes and plasma and To provide evidence for the selection of laborany specimen typs.Methods The whole blood specimens of 423 hospitalized and outpatient patients suspected of EB virus infection in Affiliated Hospital of Yan′an University were collected by the fluorescence quantitative PCR method.The quantitative detection of EB virus DNA in peripheral blood leucocytes and the quantitative detection of EB virus DNA were done respectively.The Results were compared and analyzed.Results Among the 423 specimens, the positive rate of EB virus DNA in peripheral blood white blood cells was 32.38%(137/423), the positive rate of EB virus DNA in plasma specimens was 2.36%(10/423), and the difference was statistically significant (P<0.05).In the same positive specimens, the content of EB virus DNA in peripheral blood leucocytes was higher than that in plasma.The virus content in peripheral blood leucocytes is greater than 105, and the plasma is positive.Conclusion The amount of EB virus nucleic acid load in peripheral blood leucocytes and plasma is significantly different between the two.The positive rate of peripheral blood leucocytes detection is higher than that in plasma, and the peripheral blood leucocytes is more sensitive and can reduce the missed diagnosis.The amount of EB virus in peripheral blood leucocytes reaches a certain level, which can only be detected in plasma samples.
