内镜甲状腺切除术的临床经验总结（附23例报告）

目的总结腹腔镜乳晕入路甲状腺切除术的临床应用经验。方法对23例甲状腺肿瘤患者行腹腔镜下乳晕入路手术切除。结果23例无中转传统手术病例，均手术顺利，行一侧腺叶切除22例，另1例附加峡部切除。平均手术时间115（85～230）min，平均出血量60（50—100）ml，术后无声音嘶哑、呛咳、大出血等严重并发症。2例术后出现胸骨切迹处皮下积液，经抽液和加压包扎后治愈。结论掌握腔镜甲状腺手术的指征，精细操作，采用内镜手术治疗甲状腺疾患是安全可行的，且具有较好的美容效果。

Trans - Mammary approach for endoscopic thyroidectomy （ report of 23 cases）

Objective To summarize the experience of laparoscopic thyroidectomy via anterosternal approach. Methods Twenty - three cases of thyroid tumor underwent laparoseopic thyroidectomy via anterosternal approach. Results All the 23 cases were done successfully including unilateral thyroidectomy in 22 cases and additional isthmuseetomy in 1 case. Average operative duration was 115 rain, average blood loss was 60 ml. No hoarseness, bucking, massive hemorrhage occurred postoperatively, and 2 cases had subcutaneously hydrops near sternal notchand cured by needle aspiration and pressure dressing. Conclusions With adherence to the indications of endoscopic thyroidectomy and operating with fine technique, endoscopic thyroidectomy is safe for the treatment of patients with thyroid disease and has a more favorable cosmetic effect.
