初发过敏性紫癜患儿免疫功能变化及其临床意义

摘    要:目的探讨初发过敏性紫癜(HSP)患儿细胞免疫、体液免疫功能变化及其临床意义。方法采用流式细胞术检测156例HSP患儿外周血T淋巴细胞CD3^+、CD3^+CD4^+、CD3^+CD8^+、CD3^+CD4^+CD8^+,B淋巴细胞CD3^-CD19^+及NK细胞CD3^+CD16^+CD56^+的表达水平,采用散射免疫比浊法检测血清免疫球蛋白IgG、IgA、IgM,以及补体C3、C4的水平,采用免疫化学发光法检测IgE水平,并与25例健康儿童进行比较。结果HSP患儿CD3^+、CD3^+CD4^+百分比及CD4^+/CD8^+比值均显著下降,CD3^-CD19^+百分比显著上升,与健康儿童相比差异均有统计学意义(P〈0.01),CD3^+CD4^+CD8^+百分比上升(P〈0.05),CD16^+CD56^+百分比下降(P〈0.05)。HSP患儿的IgA、IgE、IgG、C3水平升高,与健康儿童比较差异有统计学意义(P〈0.05)。结论HSP患儿存在细胞免疫及体液免疫水平失调造成的免疫功能紊乱,淋巴细胞亚群、免疫球蛋白及补体的检测对于从细胞免疫及体液免疫角度了解患儿的免疫功能,探讨疾病的发病机制、辅助诊断及预后有重要意义。

Abstract:Objective To discuss the change of cellular immune and humoral immune function in children patients with Henoch- Schonlein purpura(HSP) and to investigate its clinical significance. Methods The expression levels of T lymphocyte CD3^+ ,CD3^+ CD4^+ ,CD3^+ CD8^+ ,CD3^+ CD4^+ CD8^+ ,B lymphocyte CD3^-CD19^+ and NK lymphocyte CD3^-CD16^+ CD56^+ in peripheral blood of 156 patients with HSP was detected by flow cytometry. The level of immunioglobins(IgG,IgA,fgM) and comptement(C3,CA) of serum were evaluated by nephelometry immunoassay. The level of immunioglobin IgE was measured by immunoehemiluminometric assays and compared with 25 healthy controls. Results Compared with control group, the ratio of CD4^+/CD8^+ and the contents of CD3^+ ,CD4^+ cells in HSP patients were decreased significantly(P〈0.01),while the content of CD19^+ was significantly increased (P〈0.01). The content of CD16^+ CD56^+ was decreased(P〈0.05), while the content of CD3^+ CD4^+ CD8^+ was increased(P〈 0.05) ,the content of CD8^+ had no obvious change(P〈0.01). The level of IgA and IgE were increased significantly(P〈0.01). The level of IgG and C3 were increased (P〈0.05) ,The level of IgM and C4 had no obvious change(P〉0.05). Conclusion The cellular immune and humoral immune function in children with HSP are in disorder. To detect lymphocyte subsets, immunioglobin and complement will be of important significance to understand the immune function from the perspective of the cellular immune and humoral immune to explore the pathogenic mechanisms and to assist diagnosis and patient's prognosis.
