下齿槽神经缺失对下颌骨骨折愈合影响的实验研究

摘    要:目的观察兔下颌骨骨折愈合中下齿槽神经缺失对骨痂血管生成和矿化成骨的影响,探讨神经在颌骨骨折愈合的作用机制。方法40只新西兰白兔分作失神经下颌骨骨折的实验组和单纯下颌骨骨折的对照组,分别于术后4d、7d、14d和28d获取骨痂,通过HE染色、免疫组化对标本行形态学观察,对比两组CGRP、VIP表达情况;通过Ⅷ因子相关抗原染色检测骨痂早期新生血管密度,四环素免疫标记法检测骨痂后期矿化成骨量。结果实验组和对照组骨痂存在明显的形态学差异,实验组中CGRP、VIP表达低于对照组;实验组早期血管密度和后期矿化成骨量均低于对照组。结论下齿槽神经缺失影响骨痂的血管生成和矿化成骨量,不利于下颌骨骨折愈合。

Affection of IAN Absence on the Healing of Mandibular Fracture

Abstract:Objective To discuss the affection of nerves in the healing of jaw bone fracture by observing the affection of IAN absence on vessel formation and bone mineralization in bony callus in the healing of mandibular fracture. Methods 40 rabbits were divided into 2 groups: experiment group (n = 20) with mandibular fracture and IAN absence and control group (n = 20) with mandibular fracture only; bony callus were taken from both groups 4 days,7 days, 14 days and 28 days after operation respectively,morphological observation was made through HE staining and immunohistochemical staining,the expressions of CGRP and VIP in both groups were compared; factor Ⅷ related antigen was applied in detecting early vessel density of bony callus, tetracycline immunological marking was applied in detecting the later bone mineralization of bony callus. Results There existed an obvious morphological difference of bony callus between the 2 groups, the expressions of CGRP and VIP, the early vessel density and later bone mineralization in experiment group were lower than those in control group. Conclusion IAN absence has affection on the vessel formation and bone mineralization of bony callus and is unfavorable for the healing of mandibular fracture.
