不同程度弥漫性脂肪肝患者肝动脉血流动力学变化

目的研究不同程度脂肪肝患者肝动脉血流动力学指标的变化。方法选择我院收治的脂肪肝患者75例,其中轻度、中度、重度组各25例,另选同期健康体检者25例(对照组),采用彩色多普勒超声测定肝动脉的收缩期峰值血流速度(PSV)、舒张末期血流速度(EDV)及阻力指数(RI)。结果4组受检者的PSV、EDV、RI间差异均有统计学意义(P<0·05);两两比较,PSV对照组、轻度组与重度组间差异均有统计学意义(P<0·05),EDV除对照组与轻度组间差异无统计学意义外,其余各组间差异均有统计学意义(P<0·05),RI对照组和轻度组与中、重度组间差异均有统计学意义(P<0·05)。结论彩色多普勒超声检测脂肪肝肝动脉血流频谱,表现为随着脂肪肝程度加重,肝动脉PSV减低,EDV增高,RI减低。此种改变对评价脂肪肝的病变程度有一定价值。

Hepatic Artery Haemodynamic Changes in Different Degrees of Diffuse Fatty Liver Infiltration

Objective To study the changes of hepatic artery resistance index of various degrees of diffuse fatty liver infiltration.Methods Seventy-five cases with diffuse fatty infiltration,of whom there were 25 mild patients,25 moderate patients and 25 severe patients,were examined using color Doppler sonography in our hospital.25 healthy subjects were enrolled as controls in the meantime.Haemodynamic indexes such as peak systolic velocity (PRV),end-diastolic velocity(EDV) and resistance index (RI) were determined.Results There were significant differences in PSV,EDV and RI in four groups (P<0.05).The difference in PSV between the control group,the mild group and the severe group was significant (P<0.05).Significant difference was found in EDV between all the groups except between the control group and the mild group(P<0.05).The control group and the mild group were significantly different from the moderate group and the severe group in RI(P<0.05).Conclusion Color-doppler detection of hepatic arterial flow in fatty liver displays a decrease of hepatic artery PSV and RI and an increase of EDV.This change is of some value in evaluating the degree of the pathological changes of fatty liver.
