骨原发性血管内皮瘤

目的：本文探讨骨血管内皮瘤X线平片及CT表现并与病理分级相对照,试图找出病理分级与X线征象的相关性,方法：回顾性分析了6例经病理证实的骨血管内皮瘤,男4例,女2例,年龄分别为3－32岁,病程3－5年,结果：6例中多灶3例,单灶3例,5例为溶骨性破坏且以髋腔为主,仅1例髓腔皮质同时累及,无论病灶大小,单灶,多灶,病变周围均出现程度不等的反应性硬化,5例破坏区内残留骨,3例病灶内残留呈“发团样”或“细网状”结构,具有普通骨血管瘤的特征性表现。结论：骨血管内皮瘤在骨肿瘤分类中属于中间中低度恶性,因此本组6例X线征象与病理对照分析显示二者之间有一定相关性,但最后诊断仍依赖于组织学检查。

Primary hemangioendothelioma of Bone

Objective: To study the X-ray film and CT scan appearances of hemangioendothelioma, comparing them with the pathological features in order to find out the correlation between the pathological grade and the X-ray demonstrations. Methods: Six cases of hemangioendothelioma were analyzed, which were confirmed by pathological examination. The patients of 4 males and 2 females ranged from 3 to 32 years old. The period of the disease lasted from 3 to 5 years. Results: Of the 6 cases, single focus were found in 3 cases, multifocus in 3 cases. Osteolytic lesion were demonstrated in 5 cases, which involved of primarily in marrow, and marrow and cortex are both affected in 1 case. Various reactive osteosclerosis were found surrounding the lesion in independent of the size, single focus or multifocus of the lesion. In 5 cases, the residual bone was found in destruction areas. But the characteristic" honeycomb" or "web-like" pattern of the hemangioma were seen in 3 cases. Conclusion: The hemangioendothelioma is classified as the intermediate grade or lower grade malignaut tumor, therefore, some positive correlations between the X-ray signs and the pathological characteristics were shown in 6 the cases, but the pathologic biopsy is very necessary for final diagnosis.
