Application research of pharmaceutical care network Europe(PCNE)classification system in the physician-pharmacist joint clinic of cardiology

Objective:To investigate the application of pharmaceutical care network Europe(PCNE)classification system in identifying and solving drug related problems(DRPs)in the physician-pharmacist joint clinic of cardiology.Methods:Patients who were hospitalized for PCI in the cardiology ward in our hospital from May 1st 2018 to December 31st 2019 and subsequently visited the physician-pharmacist joint clinic for follow-up were collected.Problem,cause,intervention type,intervention acceptability,and solution state of DRPs were analyzed.Results:A total of 67 patients and 116 DRPs(1.73 DRPs per patient)were enrolled in this study.The main problem of DRPs was treatment effectiveness with a total of 82(70.7%).The main cause of DRPs was drug selection with a total of 65(56.0%).The main type of intervention was drug with a total of 104(44.8%).A total of 116 interventions were provided to physicians and patients by pharmacists,in which 111 interventions were accepted with the overall acceptance rate of 95.7%.A total of 90(77.6%)DRPs were resolved.Conclusion:Pharmacists carried out standardized closed-loop management of DRPs by PCNE classification system in physician-pharmacist joint clinic of cardiology in our hospital,and improved the efficiency of identifying and solving DRPs,and ensured the rational drug use of patients as well as reflected the professional value of pharmacists.
