雄激素相关肝腺瘤的18F-FDG PET/CT显像分析

摘    要:目的评价18F-FDG PET/CT对雄激素相关肝腺瘤的诊断价值。资料与方法收集经病理证实的肝腺瘤患者5例,分析其18F-FDG PET/CT表现。结果5例肝腺瘤共25个病灶,平均直径(3.41±1.26)cm;其中8个病灶(直径>3.5 cm)伴瘤内出血,边缘区域FDG代谢程度接近或略高于周围正常肝实质,中心出血区无FDG摄取;17个无出血肝内病灶呈低密度、FDG代谢程度呈低/等代谢;肝腺瘤最大标准化摄取值1.96±1.02。结论雄激素相关肝腺瘤的特征性PET/CT表现为易合并瘤内出血,其边缘区域代谢高于肝脏,不伴出血的肝腺瘤表现为均匀性低代谢或等代谢,功能融合影像是增强CT的辅助检查,对病变定性有重要的诊断价值。

Abstract:Purpose To evaluate the diagnostic value of 18F-FDG PET/CT for androgen-related hepatocellular adenoma.Materials and Methods 18F-FDG PET/CT imaging features of five patients with hepatocellular adenoma confirmed by pathology were analyzed retrospectively.Results There were 25 lesions in 5 patients,with the average diameter(3.41±1.26)cm.Among them,8 lesions(diameter>3.5 cm)were associated with intratumoral bleeding and necrosis,which showed mild or moderate FDG accumulation in the marginalarea,but no FDG uptake in the central area.The other 17 lesions without intratumoral bleeding showed slightly low density and relatively lower or close to the background radioactivity of the liver,the maximal SUV for hepatocellular adenoma were 1.96±1.02.Conclusion The characteristic PET/CT features of androgen-related hepatocellular adenomas is prone to intratumoral hemorrhage,and its marginal area metabolism is higher than that of the liver.Hepatocellular adenomas without hemorrhage show uniform low metabolism or isometabolism.Functional fusion imaging is the auxiliary examination of enhanced CT,and it has an important diagnostic value for the qualitative disease.
