跗骨窦入路克氏针内固定治疗跟骨骨折

目的探讨跗骨窦入路克氏针内固定治疗跟骨骨折的疗效。方法采用跗骨窦入路切开复位克氏针内固定治疗36例跟骨骨折患者。记录术前和术后6个月B?hler角及Gissane角。采用AOFAS踝-后足评分标准评价临床疗效。结果患者均获得随访,时间8~20个月。无切口感染、皮肤坏死及骨不愈合等并发症发生。B?hler角、Gissane角术后6个月均较术前明显增大(P<0.05)。末次随访时采用AOFAS踝-后足评分评价疗效:优25例,良8例,可2例,差1例,优良率为91.7%(33/36)。结论跗骨窦入路克氏针内固定治疗跟骨骨折疗效可靠,具有创伤小、切口并发症少、距下关节面暴露充分、操作简便等优势。

Treatment of calcaneal fractures with Kirschner wire fixation via sinus tarsi approach

Objective To investigate the curative effects of sinus tarsi approach Kirschner wire fixation for treatment of calcaneal fractures.Methods The 36 cases of calcaneal fractures were treated by open reduction and Kirschner wire internal fixation with sinus tarsi approach.The B?hler angle and Gissane angle were recorded at the preoperation and 6 months after surgery.The American Orthopaedic Foot and Ankle Society(AOFAS)ankle-hindfoot scoring criteria was used to assess the curative effect.Results The patients were followed up for 8~20 months,without incision infection,skin necrosis and nonunion etc complications.The calcaneal B?hler angle and Gissane angle at 6 months postoperation were significantly increased,compared with that before operation(P<0.05).At the last follow-up,according to AOFAS ankle-hindfoot score criterion,25 cases were excellent,8 cases good,2 cases fair,and 1 poor,with the excellent and good rate of 91.7%(33/36).Conclusions Sinus tarsi approach Kirschner wire fixation is a viable option for calcaneal fractures,which have the advantages of less trauma,fewer incisional complications,sufficient exposure of calcaneal subarticular surface and easy operation.
