急性胰腺炎时超声观察网膜囊病变的准确性及临床意义

目的：了解急性胰腺炎（AP）时超声观察网膜囊病变的准确性及其临床意义。方法：回顾分析了本院三年中627例AP的超声检查资料，其中100例行110次CT检查，通过与CT比较分析超声诊断网膜囊病变的准确性；与临床资料比较分析AP时网膜囊出现病变的意义。结果：超声诊断网膜囊积液的敏感性、特异性及准确性分别为62.5%、91.5%、87.3%；诊断网膜囊坏死组织则分别为87.5%、97.1%、96.4%。在轻症胰腺炎中，网膜囊积液及坏死组织的出现率为18.1%，1.8%；在重症胰腺炎中，则为58.4%及29.6%。有网膜囊积液组平均住院日为26.9天，而无网膜囊积液组为14.8天。结论：网膜囊是胰周坏死组织最常出现的部位；网膜囊积液与AP的轻重及住院时间长短有关；超声能较准确地诊断网膜囊积液及坏死组织。

Acuracy and Clinical Significance of Lesser Omentum Sac Abnormality Observed by Ultrasound in Acute Pancreatitis

Objective:To determine the acuracy and clinical significance of lesser omentum sac abnormality observed by ultrasound in acute pancreatitis (AP).Methods:Six hundred and twenty seven AP patients examined by ultrasound were studied retrospectively during Jan 1996 to Dec 1998 One hundred cases accepted CT scan Results:The sensitivity, specificity and accuracy of ultrasound detecting lesser omentum sac fluid collection were 62 5%,91 5% and 87 3% respectively , and for tissue necrosis were 87 5%,97 1% ,96 4% respectively.In mild AP,the incidence rate of fluid collection and tissue necrosis were 18 1% and 1 8% while in severe AP were 58 4% and 29 6% respectively.The hospital period was quite different for patients with and without fluid collection,26 9 and 14 8 days respectively Conclusions:Lesser omentum sac fluid collection and tissue necrosis can be detected by ultrasound accurately.The most common site of peripancreatic necrosis was lesser omentum sac.The fluid collection of lesser omentum sac was closely related to both the severity of AP and the hospital period.
