内镜逆行胰胆管造影和内镜乳头括约肌切开术对急性胆源性胰腺炎的临床应用

目的探讨内镜逆行胰胆管造影（ERCP）和内镜乳头括约肌切开术（EST）在治疗急性胆源性胰腺炎（ABP）中的临床价值。方法63例ABP患者在人院72h内行ERCP和EST治疗（内镜治疗组），64例ABP患者采用内科常规综合治疗（对照组），观察两组患者血清淀粉酶恢复时间、住院天数和住院费用。结果内镜治疗组患者的血清淀粉酶恢复时间、住院天数和住院费用均低于对照组（P〈0．05）；且两组不同病因上述指标比较差异也有统计学意义（P〈0．05）。结论ERCP和EST能缩短ABP患者的血清淀粉酶恢复时间、住院天数，且能减少住院费用，值得临床推广。

The clinical value of endoscopic retrograde cholangiopancreatography and endoscopic sphincterotomy treating acute biliary pancreatitis

Objective To explore the clinical value of endoscopic retrograde cholangiopancreatography(ERCP)and endoscopic sphincterotomy(EST) treating acute biliary pancreatitis(ABP). Methods Sixty-three patients with ABP were treated by ERCP and EST (endoscopic treating group), and sixty-four ABP patients were treated by normal methods(control group). The decrease of serum amylase level, the days and cost of hospitalization were observed in all patients. Results The decrease of serum amylase level,the days of hospitalization and the cost of hospitalization in endoscopic treating group were obviously lower than those in control group(P<0.05), and statistics difference also existed between the different etiological factors of two groups in above markers. Conclusions ERCP and EST could lower the decrease of serum amylase level, the days of hospitalization, and the cost of hospitalization of the patients with ABP. It is considered to spread in clinical practice.
