摘    要:目的探讨经尿道前列腺电切术(TURP)治疗高危、超大前列腺增生的效果。方法采用经尿道电切术治疗高危、超大前列腺增生63例,年龄72~88岁,前列腺平均重量120 g。结果平均手术时间为170 min,术中出血量平均160 ml。电切前列腺组织平均75 g。发生TURS先兆2例,无手术死亡病例。IPSS评分由术前28.4分降为5.8分,QOL由4.8分降为1.7分,最大尿流率由6.4 ml/s升为18.1 ml/s,残余尿量由128 ml降为38 ml。平均随访6个月,患者生活质量明显提高。结论对于高危、超大前列腺增生患者,TURP术是安全、有效的。

Experience on TURP in the treatment of high - risk huge benign prostate hyperplasia

Abstract:Objective To research the clinical effect of TURP in the treatment of high-risk huge benign prostate hyperplasia.Methods 63 cases huge BPH patients with high-risk were treated by TURP,whose age ranged from 72 to 88 years old.The mean weight of the prostate was 120 gram.Results The mean operation time was 170 minutes,the mean interoperation bleeding was 160 milliliter,and the resection tissues weighed 75 gram in average.Two cases suffered from the early TURS syndrome.No death occurred.IPSS decreased from 28.4 to 5.8.QOL decreased from 4.8 to 1.7.The maximum flow rate(MFR) increased from 6.4 ml/s to 18.1 ml/s.Post-void residual urine volume decreased from 128 milliliter to 38 milliliter.After operation,the follow-up time was 6 months.And life quality of patients was obviously improved.Conclusion It is considered that TURP be an effective and safe method in the treatment of high-risk huge benign prostate hyperplasia.
