不同院校医学生临床实习情况及满意度对比与分析

目的研究我国不同医学院校之间临床实习的差异性,探讨影响医学生临床实习满意度的因素和实习医生在临床医疗环境中的角色定位,尝试性提出一些临床教育的优化方案。方法对临床医学专业本科生进行问卷调查,收集上海交通大学医学院和南京医科大学两所医学院本科生临床实习情况的相关资料运用统计方法比较两所医学院在带教情况、医学生日常工作、常见操作完成情况的区别,运用Logistic回归分析影响医学生临床实习满意度的因素。结果上海交通大学医学院医学生临床实习满意度显著低于南京医科大学,两校在带教情况、医学生日常工作、常见操作完成情况均存在显著不同,Logistic回归提示住院医师带教情况是影响实习满意度的重要因素。结论不同院校本科生临床实习情况存在较大差异,通过优化临床实习环境,加强住院医师对实习医生的带教意识和教学能力,可能有利于实现实习医生"以学习为首要目标的临床医生"的角色定位。

Comparison and Analysis of Current Practice and Medical Student Satisfaction in Clerkships of Different Institutions

Objective To study the heterogeneity of clinical clerkships between medical schools in China,explore the factors affecting clerkship satisfaction in clerkships and the roles of student physicians,and to propose several ideas on improving clinical education.Methods Questionnaires were designed to survey medical students from Shanghai Jiao Tong University School of Medicine and Nanjing Medical University about their clinical clerkships,data was statistically analyzed and compared between the two schools in terms of clinical teaching,daily routines and completion of common clinical procedures.Logistic regression was employed to determine factors affecting student satisfaction in clinical clerkships.Results Clinical clerkship satisfaction is significantly lower among students from Shanghai Jiao Tong University School of Medicine than those from Nanjing Medical University.There are significant differences in clinical teaching,daily routines and completion of common clinical procedures between two medical schools.Logistic regression showed residents teaching as a significant factor affecting clerkship satisfaction.Conclusion There is significant heterogeneity within clerkships between different institutions.Optimization of clinical environment and development of resident awareness and competence of clinical teaching is needed for optimizing the student doctor role of "primary learning physician".
