中西医结合疗法对巧囊术后慢性盆腔痛及复发的影响

目的:基于治未病理论观察中药多途径疗法联合戈舍瑞林对巧囊术后慢性盆腔痛及复发的影响。方法:将80例巧囊术后患者随机分为两组各40例。对照组予散结镇痛胶囊联合戈舍瑞林治疗,治疗组予清热化瘀方口服、保留灌肠及离子导入联合戈舍瑞林治疗。治疗3个月后,比较两组临床疗效、慢性盆腔痛程度(VAS评分)及血清TNF-α、IL-6、VEGF水平变化情况,并随访1年,比较两组的妊娠情况。结果:治疗组总有效率97.50%高于对照组75.00%(P<0.05)。治疗后,两组慢性盆腔痛VAS评分及TNF-α、IL-6、VEGF水平均低于治疗前(P<0.05),且治疗组优于对照组(P<0.05)。随访1年,治疗组妊娠结局优于对照组(P<0.05)。结论:基于治未病理论采用中药多途径疗法联合戈舍瑞林治疗巧囊术后患者,可有效提高临床疗效,减轻慢性盆腔痛的发生,抑制巧囊的复发,增加有生育要求患者的妊娠率。

Efficacy observation of integrated therapy of traditional Chinese and western medicine on chronic pelvic pain and recurrence after operation of ovarian chocolate cyst

Objective:To observe the clinical effect of TCM multi-way therapy and goserelin on chronic pelvic pain and recurrence after operation of ovarian chocolate cyst based on the theory of preventive treatment of disease.Methods:80 cases of patients after operation of ovarian chocolate cyst were randomly divided into two groups,40 cases in each group.The control group was treated with Sanjie Zhentong capsule and goserelin,while the treatment group was treated with oral Chinese herbal medicine and retention enema as well as iontophoresis and goserelin.After 3 months of treatment,the clinical efficacy,the degree of chronic pelvic pain(VAS score)and the changes of serum TNF-α,IL-6 and VEGF levels were compared between two groups.Both groups were followed up for 1 year,and the pregnancy condition was compared between two groups.Results:The total effective rate of treatment group was 97.50%,which was higher than 75.00%of control group(P﹤0.05).After treatment,VAS score and the levels of TNF-α,IL-6 and VEGF in both groups were improved compared before treatment(P﹤0.05),and the improvement of various indexes in treatment group was better than in control group(P﹤0.05).Following-up for one year showed that the pregnancy outcome of treatment group was better than that of control group(P﹤0.05).Conclusion:To apply TCM multi-way therapy and goserelin to treating the patients after operation of ovarian chocolate cyst based on the theory of preventive treatment of disease,it can effectively improve the clinical efficacy,reduce the occurrence of chronic pelvic pain,inhibit the recurrence of ovarian chocolate cyst,and increase the pregnancy rate of patients with fertility requirements.
