总体止血潜能评价妊娠期高凝状态的临床应用

目的研究正常孕妇在早、中、晚妊娠期PT、APTT、TT、FIB、OHP、OCP及OFP变化,探讨其对妊娠各期发生凝血功能异常的试验诊断意义。方法检测37例早孕期（孕期〈12周）女性、42例中孕期（孕期13-27周）女性、58例晚孕期（孕期≥28周）女性及50例正常非妊娠女性血浆PT、APTT、TT、FIB、OHP、OCP及OFP,比较不同孕期与正常对照之间各指标的差异及不同孕期各指标的变化规律。结果早孕组、中孕组、晚孕组分别与对照组比较,PT、APTT显著缩短,FIB、OHP显著升高,OFP显著降低,中、晚孕组OCP较对照组显著升高（P〈0.05）,TT无显著变化（P〉0.05）。早孕组、中孕组、晚孕组之间比较,PT、APTT、TT差异无统计学意义（P〉0.05）。晚孕组FIB与OHP较早孕组、中孕组显著升高。中、晚孕组OFP显著低于早孕组。OCP随孕期增长而不断升高（P〈0.01）。结论 OHP、OCP及OFP可反映妊娠期机体凝血能力增强及纤溶能力的降低,其敏感性优于PT、APTT、TT、FIB。

Evaluation of Hypercoagulability by Overall Haemastasis Potential in Pregnant Women

Objective To investigate the changes of PT,APTT,TT,FIB,OHP,OCP and OFP in women with different pregnant stages.Methods 37 women in early trimester of pregnancy（12 weeks）,42 in mid trimester of pregnancy（13-27 weeks）,58 in late trimester of pregnancy（≥28 weeks） and 50 healthy controls were selected into the study.PT,APTT,TT,FIB,OHP,OCP and OFP were determined.Results PT,APTT,OFP were decreased and FIB,OHP,OCP were increased in pregnant women compared to the healthy controls.Higher level of OCP in mid and late trimester of pregnancy than in healthy controls were found.Increased FIB and OHP were found in late trimester of pregnancy compared to early and mid trimester of pregnancy as well as decreased OFP in mid and late trimester of pregnancy compared to early trimester.Progressive increases of OCP were found along with the progress of pregnancy.Conclusion OHP,OCP and OFP assays are simple methods which can evaluate the increased coagulation ability and decreased fibrinolysis ability with better sentivity compared to PT,APTT,TT and FIB.
