早期强直性脊柱炎典型X线、CT表现

目的探讨早期强直性脊柱炎(ankylosing spondylitis,AS)X线及CT影像学特征,为AS的早发现及治疗提供临床依据。方法收集2006年2月～2010年10月我院确诊为早期AS的151例患者的X线及CT资料作回顾性分析,行常规骶髂关节CT扫描及脊椎正侧位、骨盆X线平片检查。结果绝大多数(147例)病例均有骶髂关节影像学改变,少数(4例)病例骶髂关节则无AS变化,表现为颈椎改变。结论 AS早期发病一般表现上行性或下行性,占绝大多数的上行性病例则必先侵犯骶髂关节;而少数下行性病例则首先侵犯颈椎。

The analysis of early ankylosing spondylitis with X-ray and computed to- mography

Objective To explore the features of X-ray and CT imaging manifestation of early ankylosing spondylitis. Meth- ods The X-ray and CT imaging data for 151 patient of early ankylosing spondylitis were reviewed. Results Most of the cases showed change in sacroiliac articulation, and a few cases showed no change. Conclusion The early ankylosing spondylitis showes the features of up spread or downward,it first involves the sacroiliac joint in cases of up spread and first involves the cervical in cases of downward. Cartilaginous joint,ligment,tendon and attachment of tendon are often affected, sometimes synovial joint.
