缠绕型和管型冠状动脉内支架在冠心病治疗中的应用

摘    要:目的 探讨应用缠绕型和管型冠状动脉内支架治疗冠心病的效果。方法 对 2 3例冠心病患者根据不同情况植入冠状动脉内支架 ,其中DeNovo支架 17个 ,Suboptimal支架 6个 ,Bail-out支架 2个。结果  2 3例患者 2 5支血管植入 2 5个冠状动脉内支架。其中缠绕型支架 9个 ,包括Gianturco -Robin支架2个 ,XT支架 7个 ;管型支架植入 16个 ,均为NIR支架。植入前降支 15个 ,回旋支 2个 ,右冠状动脉 8个。所有支架植入均获得成功。 3例心绞痛复发的患者经造影证实为再狭窄 ,其中XT支架 1个 ,NIR支架 2个 ,总的再狭窄率为 12 % ,缠绕型和管型支架的再狭窄率分别为 11%和 12 .5 %。结论 冠状动脉内支架为治疗PTCA术后急性冠状动脉闭塞、降低再狭窄率的有效措施 ;根据不同的病变合理选择缠绕型和管型支架可提高支架植入的成功率 ,减少并发症。

Abstract:Objective To research the effect of treatment by using the coiled and tubular coronary stents on coronary artery disease . Methods Based on different conditions, intracoronary stenting were performed in 23 patients with coronary artery disease.Among of them, there were 17 de novo stenting, 6 suboptimal stenting and 2 bail-out stenting. Results Coronary stents were implanted in 23 patients into 25 branches of coronary artery . There were 9 coiled stents including 2 Gianturco-Robin stents and 7 XT stents, 16 tubular stents were all the NIR stents. Fifteen stents were implanted into LAD, 2 stents into LCX, 8 stents into RCA. All the coronary stents were implanted successfully.Three patients had an attack of angina pectoris, and restenosis were conformed by coronary artery angiography, in which 1 case was XT stent, the others were NIR stents. The total restenosis rate was 12%, the coiled and tubular stent's restenosis rates were 11% and 12.5% respectively. Conclusions Intracoronary stenting was the efficient measure in treating acute coronary artery closure after percutaneous transluminal coronary angioplasty(PTCA) and reducing restenosis. Making rational use of the coiled and tube stents could raise the success rate of intracoronary stenting, and reduce the complications.
