原因不明复发性流产主动免疫治疗中三种免疫原的疗效比较

目的探讨采用不同免疫原的淋巴细胞进行主动免疫治疗原因不明复发性流产(unexplained RSA,URSA)的安全性和有效性。方法选择1995年1月至2006年12月在上海交通大学医学院附属仁济医院就诊260例URSA患者进行主动免疫治疗,免疫原根据临床情况采用三种:丈夫的淋巴细胞、男性或女性无关第三个体的淋巴细胞。结果除外胚胎绒毛染色体异常的病例(10例)所得纠正妊娠成功率:接受丈夫的淋巴细胞成功率为87.28%(151/173);接受男性无关第三个体淋巴细胞为75.81%(47/62);接受女性无关第三个体淋巴细胞为86.67%(13/15)。三者比较差异无统计学意义(P>0.05)。总成功率为84.40%(211/250),同时采用三种免疫原免疫均未见明显副反应。结论对URSA患者采用不同免疫原的淋巴细胞进行主动免疫,具有相似的疗效和安全性。

The selection of immunogens in the active immunotherapy of URSA

Objective To investigate the efficacy and safety of the active immunotherapies of URSA with three distinct immunogens.Method 260 URSA women were received the active immunotherapies with the paternal lymphocytes,the male third party lymphocytes and the female third party lymphocytes.Results There were 211 live birth in 250 pregnancy(exclude 10 pregnancy loss with fetal chromosome aberration),the corrected live-birth rate was 84.4%.The corrected live-birth rates of active immunotherapies with three distinct immunogens were:87.28%(151/173)in the paternal lymphocytes group,75.81%(47/62)in the male third party lymphocytes group,86.67%(13/15)in the female third party lymphocytes group.There was no statistic difference significantly among the corrected live-birth rates of the active immunotherapies in three distinct immunogens(P>0.05).There were no case of obvious adverse effects to be found.Conclusion The efficacy and safety of the active immunotherapies of URSA with three distinct immunogens are similar.
