男性乳腺发育症的超声分析

摘    要:目的探讨男性乳腺发育症的声像图特征及其超声诊断价值。方法41例(50个乳房)男性乳腺肿块患者行高频超声检查,观察乳腺腺体组织形态和内部回声,测量病变大小、范围,记录病变的彩色多普勒血流情况。结果48个乳房超声诊断为男性乳腺发育症,2个诊断为乳腺占位。均行手术,超声定位与手术完全一致。48个乳房超声诊断与病理相符,诊断准确率为96%。声像图表现为28个以乳头为中心深面不均质的低回声,低回声内未见明显腺体回声;20个为腺体组织均匀性增厚,不均质回声区内可见多个不规则片状低回声及条状高回声,另2个为低回声伴蟹足样改变。彩色多普勒显示38个无明显血流信号,10个仅有少量血流信号,2个低回声区内可见丰富血流信号。结论男性乳腺发育症具有较为典型的声像图特征,彩色多普勒超声对其诊断、手术指征的选择、术后随访及非手术治疗动态观察均具有较高临床应用价值。

Abstract:Objective To explore the sonogram features and diagnostic value of uhrasonography for gynaecomastia. Methods Fifty breasts in 41 male patients with breast masses were examined by high frequency ultrasound. Then the tissue morphous and internal echo of mammary glandular organ were observed,lesion size and range were measured, and the condition of CDFI was recorded. Results Forty-eight breasts were diagnosed as gynaecomastia by ultrasonography, 2 cases as breast tumor. Operation was performed on all the cases and the ultrasonic results were accordant to the surgery. Forty-eight cases of gynaecomastia were confirmed by pathology, the diagnostic rate was 96%. The sonogram displayed inhomogeneous low level echo in 28 breasts, no obvious glandular organ echo was found in low level echo; glandular organ tissue was thickening, irregular lamellar low level echo and cord-like high level echo were found in heterogeneous echopattern area in 20 breasts, and low level echo with pea crab foot changes was found in 2 breasts. No blood flow signals were found in 38 breasts by CDFI, a little blood flow signal in 10 breasts,and abundant blood flow signals in low echo area in 2 breasts. Conclusion Gynaeeomastia has typical sonogram features. Color Doppler ultrasound has high clinical application value in diagnosis, selection of operative indication, postoperative follow-up and dynamic observation of non-operative treatment.
