49例中药注射剂输液反应原因分析及应对策略

目的探讨应用中药注射剂发生输液反应的原因及配置应对策略。方法对广州医学院第二附属医院在2004年1月-2008年12月期间发生的中药注射剂发生输液反应案例进行年龄、药物配置环境、药物剂量、输液速度等因素的回顾性分析。结果年龄〉50岁的患者中药注射剂输液反应发生率高于≤50岁组（χ2=16.486,P〈0.01） 医院配置中心（局部100级环境）配药的输液反应发生率低于治疗室配药（χ2=82.850,P〈0.01） 2例超规定用量发生输液反应,1例输液速度过快发生输液反应。结论高龄患者、开放环境下配置药物、超剂量用药和输注速度过快易导致中药注射剂输液反应,值得临床重视。

Reasons Analysis and Management Strategies for 49 Cases of Adverse Reactions of Chinese Herbal Injections

Objective To explore the reasons and management strategies of adverse reactions to Chinese herbal injections.Methods Retrospective analysis were taken on all adverse reactions to Chinese herbal injections happened in our hospital between Jan.2004 and Dec.2008.Results There were higher infusion adverse reactions in 50 year-old group than ≤50 year-old group（86% vs 14%,χ2=16.486,P0.05）;The higher infusion adverse reactions also happened more easily in the situation the infusion prepared from opened environment than in the one the infusion prepared from compound preparation center（100-grade luminar air flow conditions）,χ2=82.850,P0.05;too rapid infusion and giving the dosage beyond the permitted limit easily caused the infusion adverse reactions.Conclusion Great attention must be paid to that the infusion adverse reactions easily occur in elderly patients,in the situation the infusion prepared from opened environment,and when too rapid infusion was given and the dosage was
