调强放疗同步化疗治疗食管癌的近期疗效观察

目的观察常规放疗和调强放疗联合PF方案同期放化疗治疗食管癌的近期疗效和毒副作用。方法常规组（A组）31例采用常规照射方法，6／8MV高能X线，总剂量60—70Gy；调强组（B组）31例放疗均采用6／8MV—X线。总剂量66～70Gy。两组均放疗第1天配合PF方案化疗，21天1个周期，连续2个周期。结果常规组和调强组总有效率分别为74．19％和93．55％（P〈0．05）。常规组毒副作用大于调强组，骨髓抑制、胃肠道反应及放射性食管炎发生率差异有显著性差异（P〈0．01）。结论调强放疗联合PF方案治疗食管癌较常规放疗联合PF方案有很好的近期疗效，毒副作用显著减轻，晚期并发症的发生率及远期疗效有待进一步观察。

The short-term result of intensity modulated radiation plus chemotherapy in treating esophageal carcinoma

Objective To observe the short-term result and toxicity of intensity modulated radiation （IMRT） plus chemotherapy and conventional radiotherapy plus chemotherapy in the treatment of esophageal carcinoma. Methods Totally 62 patients were enrolled in this group. Group A （ n = 31 ） received conventional radiotherapy to a total dose of 60 - 70Gy and were given extemal beam radiation of 6/8mvx - ray, and group B （n = 31 ） received IMRT to a total dose of 66 -70Gy and were given external beam radiation of 6/8mvx -ray. All patients were all combined with chemotherapy. The effcicacy and side effects were assessed after two cycles of chemotherapy. Results The overall response rate was 74. 19% in group A and 93.55% in group B （P 〈0. 05） The acute toxic effect was significantly sevever in group A than in group B, which included myelosuppression, gastrointestinal toxicities, and radiation esphagitis, were significantly severer in group A than in group B （P 〈0. 01）. Conclusion IMRT plus chemotherapy has shown satisfactory efficacy and safety in treating esophageal carcinma. Complications and long-term survival require further study.
