伴有精神症状癫痫的立体定向手术治疗

目的探讨立体定向技术对伴有精神症状癫痫的治疗效果。方法采用立体定向技术,运用多元化定位和多靶点组合,射频热凝治疗伴有精神症状的癫痫病人13例。采用癫痫的国内疗效评定标准和全国精神外科疗效评定标准进行疗效评定。结果手术治疗1年后,13例病人中,癫痫症状基本控制,9例癫痫未再发作,4例发作次数减少50%以上;11例精神病症状基本消失,2例部分症状存在。结论双侧杏仁核、双侧内侧隔区及单侧Forel-H区多靶点组合治疗,对伴有精神症状癫痫的治疗有明显效果。

Therapeutic effect of stereotactic surgery on epileptic with psychiatric disorders

Objective To explore the therapeutic effect of stereotactic surgery on patients with epilepsy and psychiatric disorders.Methods 13 patients were treated by stereotactic techniques,multi-element location and multi-nucleus lesion by radio-frequency equipment.The therapeutic effect were estimated using national standard.Results One year later,the epilepsy was controlled in 9 patients and reduced more than 50% in 4 patients,the psychiatric disorders disappeared in 11 cases and improved in 2 cases.Conclusion The lesion of bilateral amygdala,bilateral medial septal area and unilateral Forel-H area might be an effective treatment on epileptic with psychiatric disorders.
