膝关节前交叉韧带重建术后MRI评价

目的 前瞻性分析膝关节前交叉韧带（ACL）重建术后1年内移植物在MRI形态、信号的改变与组织学改变的相关性。方法 34例ACL重建术后患者，以术后1、3、6、9、12个月为随访时间点进行MRI检查，观察移植物在不同时间的形态和信号变化，分别测量增强前后移植物、髌下、移植物周围滑膜的信号强度，计算其信噪比，并分别进行组间和组内比较。结果 ACL重建术后移植物信号逐渐增强，术后9个月时信号最强，1年后信号强度有下降。结论 MRI尤其是增强MRI能反映膝关节移植物正常韧带化过程中的形态学和组织学变化。

Diagnostic Value of MRI in Anterior Cruciate Ligament Reconstruction of Knee Joint

Objective To prospectively study the normal appearance of the reconstructed anterior cruciate ligament (ACL) on MRI during the first postoperative year, and to assess the diagnostic value of contrast-enhanced MRI.Methods Sixty-one MRI examinations were performed in 34 patients after ACL reconstruction with semitendinosus and gracilis autograft. These individuals were examined cliniclly and with MRI at 1, 3, 6, 9 months and 1 year after operation. These patients with clinical instability were excluded. Changes over time in the signal/noise ration at midsubstance with and without administration of GD-DTPA were analysed and compared with the Wilcoxon test. The same measurements were made about the soft tissuses of the infrapatellar fat pad and the synovial tissues surrounding the graft. Results During the fisrt year after operation there was a significant change in signal intensity of the neoligament, and the highest signal intensity was found at 9 months. Conclusion Changes in MRI signal intensity reflect the process of graft remodeling.
