糖尿病患者病情与泪膜改变的相关性研究

目的探讨糖尿病病程、血糖控制状况以及糖尿病视网膜病变与眼表泪膜功能改变的相关性。方法分别对50例2型糖尿病患者共100眼进行荧光素染色(FL),泪膜破裂时间(BUT)测定和基础泪液分泌(Schirmer-I)试验,将检测结果与糖尿病病程、血糖控制状况以及糖尿病视网膜病变情况进行相关性分析。结果BUT、FL积分、Schirmer-I试验结果与糖尿病病程、血糖控制状况以及糖尿病视网膜病变程度均具有显著相关性(P<0·01);BUT、Schirmer-I试验与糖尿病病程、血糖控制状况以及糖尿病视网膜病变呈负相关性,FL积分与糖尿病病程、血糖控制状况以及糖尿病视网膜病变呈正相关性;其中BUT、FL积分与糖尿病病程、及糖尿病视网膜病变的相关性更显著。结论糖尿病患者存在泪膜功能的损害,糖尿病病程、血糖控制状况和糖尿病视网膜病变的情况直接影响泪膜功能的损害的程度。

The Study of the Relativity between Diabetes Meilitus and Tear Film Function

Objective To investigate the correlation of the duration, severity and grade of diabetic retinopathy (DR) with the changes of tear film function in diabetic patients.Methods Fifty patients (100 eyes ) with non insulin-dependent diabetes mellitus underwent examinations including tear film break up time (BUT) and fluorescein staining (FL), Schirmer-I test. Results The BUT value,Schirmer-I test value and the score of FL were significantly related to the duration of diabetes mellitus,the control condition of diabetes and the grade of DR (P<0.01) .The BUT value and Schirmer-I test value were in negative correlation with the duration of diabetes mellitus, the control condition of diabetes and the grade of DR, the score of FL was in positive correlation with the duration of diabetes mellitus,the control condition of diabetes and the grade of DR. The BUT value and the score of FL had an even higher correlation with the duration of diabetes mellitus and the grade of DR.ConclusionDiabetes mellitus would damage tear film function and the extent of damage was closely related to the duration and the control condition of diabetes,and the grade of DR.
