不同方式子宫切除126例分析

目的：评估不同方式子宫切除在不同子宫病变中的临床运用价值。方法：应用腹腔镜协助下行阴式子宫切除(LAVH)54例，腹腔镜筋膜内子宫切除(CISH)32例，与传统经腹子宫切除(TAH)40例，进行回顾性比较分析。结果：三种术式和不同子宫疾病在手术时间、术中出血量、术后肛门排气时间、术后3天体温、术后住院天数比较均有明显差异(P＜0．05)。结论：CISH和LAVH较TAH有损伤少、恢复快的特点，是有效的子宫切除方式3子宫腺肌症宜采用LAVH，而对于功血和大多数子宫肌瘤，宜采用CISH。

A retrospective study of 126 cases with different operational methods of hysterectomy

Objective:To study the clinical value of different operational methods of hysterectomy on different uterine disease Methods:A retrospective study was made on 126 cases of three kinds of uterine disease including myoma;endometrioma and dysfunctional bleeding with 54 cases of laparoscopic assisted vaginal hysterectomy (LAVH); 32 cases of classic intrafascial SEMM hysterectomy (CISH) and 40 cases of transabdominal hysterectomy (TAH) respectively Results:Comparative study of three operational methods of hysterectomy and uterine disease was significantly different including operational time,bleeding volume of operation,the time of postoperative anus exsufflation,the temperature on the 3 rd postoperative day and the time of hostpitalization after operation ( P <0 05) Conclusion:The LAVH and CISH are effective methods of hysterectomy and featured with less injury during operation,quicker recovery after operation comparing with TAH and indicative in patient with endometrioma and dysfunctional bleeding or myoma respectively
