2型糖尿病肾病与血浆抵抗素及胰岛素抵抗的关系

目的探讨血浆抵抗素及胰岛素抵抗与2型糖尿病肾病(DN)之间的相互关系。方法选取60例临床确诊的2型糖尿病患者,按有无DN分为2组:DN组30例和无DN组30例,以30例同期体检的健康志愿者作为对照组。采用双抗体夹心酶联免疫吸附试验测定所选90例实验对象的血浆抵抗素浓度,时间分辨分析法测定胰岛素,氧化酶法测定血糖,全自动生化分析仪检测血脂。结果①DN组血浆抵抗素浓度高于无DN组及对照组(P0.05),差异有统计学意义;②60例病例相关分析得出:血浆抵抗素浓度与空腹胰岛素(FINS)、糖化血红蛋白(HbAlc)、胰岛素抵抗指数(HOMA-IR)、胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)呈正相关,与空腹血糖(FPG)、甘油三酯(TG)、体重指数(BMI)、高密度酯蛋白胆固醇(HDL-C)均无相关性。结论空腹血浆抵抗素水平在2型糖尿病有DN者中显著升高,并与2型糖尿病患者血糖控制不良的程度有关、与2型糖尿病脂代谢紊乱相关,在2型DN的发生、发展中可能起一定作用。

Relationship between Type 2 Diabetic Nephropathy and Plasma Resistin and Insulin Resistance

[ Objective] To discuss the relationship between plasma resistin and insulin resistance with type 2 diabetic nephropathy （ DN ）. [ Methods ] Totally 60 cases of type 2 diabetes were selected, and divided into 2 groups according to whether they had DN or not： 30 cases of DN group, 30 cases of non-DN group, 30 cases of healthy volunteers as control group. Double-antibody sandwich enzyme-linked immunosorbent assay was adopted to determine the plasma resistin concentration of 90 selected subjects , time-resolved method was for determination of insulin; blood glucose was determined by oxidase method; lipids was determined by automatic biochemical analyzer. [ Results] （1）Plasma resistin concentration of DN group was higher than that of non-DN group （ P 〈 0. 05 ）, the difference was significant; （2）Case Correlation analysis on 60 cases showed that there was a significant positive correlation between plasma resistin concentration and FINS, HbA1c, HOMA-IR, TC, LDL-C, and no correlation between plasma resistin concentration and FBG, TG, BMI, HDL-C. [ Conclusion ] Fasting plasma resistin level is significantly high in patient with type 2 diabetes and DN, and relates with poor control of blood glucose and lipid metabolic disorders, and may play a role in the occurrence and development of DN in patient with type 2 diabetes.
