人工椎间盘与人工髓核置换治疗腰椎间盘突出的临床比较

目的探讨腰椎间盘突出症患者行人工椎间盘与人工髓核置换的临床应用价值及差异。方法对2000年5月—2005年4月行人工腰椎间盘置换的12例与行人工髓核置换的28例患者进行比较分析。结果行人工腰椎间盘置换的12例患者平均随访17.5(6~30)月,临床评价:优9例,良2例,尚可1例;行人工髓核置换的28例患者平均随访12(4~18)月,临床评价:优19例,良5例,尚可2例,另有2例术后1个月出现假体移位并症状复发,后再次手术取出。结论腰椎人工椎间盘与人工髓核置换均可恢复椎间隙高度,且近期疗效满意,后者手术易操作,创伤小,费用低,更易为术者和患者所接受。

Analysis of Comparing Artificial Disc Replacement with Artificial Nucleus Replacement in Lumbar Intervertebral Disc Herniation

Objective To explore the difference and c1inical value between artificial disc replacement and artificial nucleus replacement in the treatment of lumbar disc herniation.Methods The comparative analysis was performed in 12 patients treated with artificial disc replacement and 28 patients with atificial nucleus replacement from August,2000 to July,2005.Results The 12 patients treated with artificial disc replacement were followed up with an average of 17.5 months(6~30 months).The clinical outcomes were excellent in 9 and good in 2 cases,and fair in one case.The 25 patients treated with artificial nuclear replacement were followed up with an average of 12 months(4~18 months).The clinical outcomes were excellent in 18,good in four and fair in two cases.And the migration of the prosthesis was found in two patient whose clinical symptoms recurred one month later after operation.Conclusion Both the artificial disc replacement and artificial nucleus replacement can restore the intervertebral height and acquire the satisfactory short-term clinical result.But the latter is more acceptable by the surgeons and patients for its easier operation,less damage and low cost.
