Short term effect of percutaneous transluminal pulmonary angioplasty in elderly patients

Objective To explore the short term safety and efficacy of percutaneous transluminal pulmonary angioplasty(PTPA)in elderly patients with chronic thromboembolic pulmonary hypertension(CTEPH).Methods CTEPH patients treated with PTPA in Zhongshan Hospital Affiliated to Fudan University from December 2017 to December 2019 were retrospectively analyzed.They were divided into two groups according to their age:non-elderly group(n=24)and elderly group(n=13).Cardiac function,6 minutes walking distance(6MWD),N-terminal pro-brain natriuretic peptide(NT-proBNP)level,echocardiography,right cardiac catheterization indexes,and complications of PTPA treatment were collected and compared at baseline and follow-up period.Results A total of 37 CTEPH patients and 90 times of PTPA treatment were included in the study.The NT-proBNP and mean right atrial pressure levels of the elderly group were higher than those of the nonelderly group(P=0.021,0.049,respectively)at baseline.During the follow-up period,the NT-proBNP level of the elderly group was still higher than that of the non-elderly group(P=0.012).Compared with baseline,PVR decreased,NT-proBNP decreased,and 6MWD increased in both groups during follow-up.The decrease of mean pulmonary artery pressure and the increase of 6MWD in the elderly group were not as good as those in the non-elderly group(P=0.049,0.005,respectively),but there was no significant difference in the decrease in PVR between the two groups(P=0.200).There was no significant difference in the incidence of pulmonary edema and pulmonary vascular injury between the two groups.Conclusion PTPA may be a safe and effective treatment for elderly patients with CTEPH.
