经颅多普勒超声在脑血栓溶栓中的作用

目的观察经颅多普勒超声监测在动、静脉溶栓治疗急性脑梗死中的指导作用。方法回顾性分析200例急性脑梗死患者动、静脉溶栓治疗的资料，200例均为大脑中动脉主干及分支栓塞，其中100例未用经颅多普勒超声监测（对照组），2008年后100例溶栓患者治疗前、后用经颅多普勒超声监测（监测组）。治疗前后收集相关数据，并做统计学分析。结果监测组大脑中动脉、大脑前动脉Vs、Vm均明显高于溶栓前，而RI、PI低于溶栓前fP〈0．05）；监测组与对照组比较．术前ESS评分差别无统计学意义（p〉0．05）；术后24h、术后2wESS评分差别有统计学意义（P〈0．051；术后脑出血、继发脑血管栓塞、脑血管痉挛、心律失常、心跳骤停并发症差别有统计学意义（P〈0．05）。结论经颅多普勒超声可以为脑血栓溶栓治疗提供指导。

Transcranial doppler ultrasound role in cerebral thrombus thrombolysis

Objective To explorer safety and efficacy of thrombolysis guided by transcranial doppler （TCD） in patients with acute ischemic stroke. Methods By analyzing retrospectively 200 patients whose middle cerebral artery or it＇s branch were embolism were treated with thrombolysis combined （IV-IA）. A total of 100 patients thrombolysis （50.0%） were routine （control group）, and 100 thrombolysis （50.0%） were guided by TCD （monitoring group）. Collect simultaneously correlated data. Results The Vs and Vm of MCA or PCA were higher, the RI and PI of MCA or PCA were lower than control group. In monitoring group, the outcome was more, the complication was juniorer. Conclusion Thrombolysis guided by TCD was feasible and safe.
