七氟醚吸入麻醉与异丙酚静脉麻醉在妇科日间手术中的临床应用比较

摘    要:目的比较七氟醚吸入麻醉与异丙酚静脉麻醉在妇科日间手术中运用的优缺点。方法选择148例ASAⅠ~Ⅱ级行妇科日间手术的患者随机分为七氟醚组(S组)和异丙酚组(P组),两组分别用七氟醚及异丙酚诱导和维持,分别记录各组患者入室时、诱导1min时、插入喉罩时的平均动脉压(MAP)、心率(HR),诱导开始至插入喉罩所用时间(T1)、停用麻醉药至拔除喉罩所用时间(T2)、停用麻醉药到达到离院标准的时间(T3),以及术后10小时内嗜睡、恶心、呕吐、头痛的发生情况。结果 S组患者诱导时血流动力学波动明显小于P组(P〈0.05),停药至拔除喉罩时间及停药至达到出院水平时间明显短于P组(P〈0.05),而术后恶心呕吐明显高于P组(P〈0.05)。结论七氟醚吸入麻醉运用于妇科日间手术过程中,具有诱导快、苏醒快,血流动力学稳定,停药至达到出院标准时间短等优点。但其术后恶心呕吐发生率较高,应引起高度重视。

Clinical comparison of sevoflurane inhalation anaesthesia and propofol total intravenous anesthesia in gynaecologic day surgery

Abstract:Objective To compare the advantages and disadvantages of sevoflurane inhalation anesthesia and propofol total intravenous anesthesia(TIVA)in gynaecologic day surgery.Methods 148 ASAⅠ~Ⅱpatients undergoing gynaecologic day surgery were randomly divided into two groups:group S and group P.Sevoflurane and propofol were used in the induction and maintenance of anesthesia respectively.The mean arterial pressure and heart rate during anesthesia were recorded.The time of induction,consciousness recovery time,hospitalization time and post operative nausea and vomiting(PONV)were recorded.Results The hemodynamics in group S was more stable than the paeients in group P(P〈0.05).The time of induction in group S was longer than the patients in group P.But the consciousness recovery time and hospitalization time in group S were obviously shorter than those in group P(P〈0.05).On the other side,the PONV in group S was more than group P(P〈0.05).Conclusion Compared with propofol,patients received sevoflurane anesthesia have stable hemodynamics,short induction time and recovery time.But the PONV should be noticed.
