同型半胱氨酸、血脂与脑血管疾病的相关性研究

摘    要:目的探讨脑血管疾病患者血清中同型半胱氨酸(HCY)、总胆固醇(TC)、甘油三脂(TG)、高密度脂蛋白胆固醇(HDL-C)和低密度脂蛋白胆固醇(LDL-C)的含量变化以及与梗死面积的相关性。方法经CT确诊的脑血管疾病患者共108例(研究组),其中脑梗死患者74例,脑出血患者34例;对照组60例。检测血清中HCY、TC、TG、HDL-C、LDL-C的含量。结果 (1)脑出血患者HCY水平为(17.61±4.06)μmol/L,脑梗死患者为(19.72±5.65)μmol/L,二者与正常对照组([9.17±2.24)]μmol/L比较均差异有统计学意义(P〈0.01);脑梗死患者TC、TG、HDL-C、LDL-C均明显升高(P〈0.05),脑出血患者仅LDL-C增高(P〈0.05),其余各项无显著性差异。(2)脑梗死不同临床分型组间血清HCY、TC、TG、HDL-C、LDL-C水平两两比较,差异无统计学意义(P〉0.05)。结论血清HCY水平升高与脑血管疾病的发生有密切关系,随着血清HCY水平升高,罹患脑梗死危险性增加;血清HCY水平并不能很好地反映脑梗死病变的程度,其升高仅是脑血管疾病发生的危险因素之一。

Abstract:Objective To study the correlation between the content change infarction area and the homocysteine(HCY),total cholesterol(TC),total triglyceride(TG),high density lipoproptein(HDL-C),Low density lipoproptein(LDL-C)in the patients with cerebrovascular disease.Methods The values of HCY,TC,TG,HDL-C and LDL-C were detected in 74 cases with cerebral infarction,34 cases with cerebral hemorrhage,and 60 negative controls.Results The values of HCY in patients with acute brain infarction and cerebral hemorrhage were remarkable higher than those in the control group (P0.01).TC,TG,HDL-C and LDL-C in the patients with cerebral infarction were significantly increased (P0.05);only LDL increased patients with cerebral hemorrhage (P0.05).HCY,TC,TG,HDL-C and LDL-C were uncorrelated significantly in the process of brain infarction.Conclusion Elevated levels of serum HCY and cerebrovascular diseases are closely related.With the increased serum HCY levels,an increased risk of suffering from cerebral infarction,but serum HCY levels may not properly reflect the extent of cerebral lesions,and the increase is only one of cerebrovascular disease risk factors.
