双氯芬酸钠对大鼠骨质疏松性骨折愈合的影响

摘    要:目的:探讨双氯芬酸钠对大鼠骨质疏松性骨折愈合的影响。方法:60只8月龄雌性SD大鼠,以卵巢去势法行骨质疏松造模,3个月后随机分为3组。A组和B组行骨折造模后分别灌胃生理盐水和双氯芬酸钠5mg·kg^-1·d^-1;C组先灌胃双氯芬酸钠5mg·kg^-1·d^-1,3周后行骨折造模。各组于骨折造模后2、3、4、6周行放射学、组织学观察及生物力学检测。结果:不同时间点B组骨折端x线灰度值低于对照组(A组)。术后第2周,C组骨折端X线灰度值低于A组,其他时间点A组和C组间无明显差异。不同时间点B组骨小梁密度值低于A组(P〈0.01)。术后第2~4周,C组骨小梁密度值低于A组(P〈0.05),术后第6周无明显差异。术后第6周B组和C组生物力学强度值低于A组。结论:双氯芬酸钠能使大鼠骨质疏松性骨折的愈合时间延迟、强度降低。骨质疏松性骨折前长时间使用双氯芬酸钠对骨折愈合有一定不良影响,骨折后应及时停药。

Abstract:Objective: To study the effect of diclofenac sodium on the healing of osteoporotic fracture in rats. Methods:Sixty female,g-month old, Sprague Dawley rats of osteoporotic models were established by ovariectomy and randomized into 3 groups after 3 months. Femurs of rats in groups A and B were fractured and then the rats were given gavage of normal saline and diclofenac sodium at the dosage of 5 mg·kg^-1 , d^-1 for 3 weeks respectively, while rats in group C were given the same dose of diclofenac sodium for 3 weeks and then their femurs were fractured. X-ray examination, histological observation and biomechanical test were performed in the 2nd, 3rd, 4th and 6th week after fracture. Results:The gray-scale value in group B was lower than that in group A(P〈0.01). There was no significant difference between gray-scale values of X-ray films in groups A and C,except in the 2nd week after fracture. The density value of bone trabeeula in group B is lower than that in group A (P〈0.01), while in group C that was lower than in group A during 2-4 weeks after fracture, but no difference in 6th week. The biomeehanieal strength in groups B and C was lower than that in group A in the 6th week. Conclusion:The healing of osteoporotie fracture in rats can be delayed and the biomechanical strength of bone is decreased by dielofenae sodium. Long-term administration of diclofenac sodium before osteoporotic fracture has inhibitory effect on its healing. It is necessary to dis continue diclofenac sodium after fracture.
