中西医结合治疗输尿管上段结石临床观察

目的：探讨输尿管上段结石在ESWL后应用草木犀流浸液片促进结石排出的疗效。方法：选确诊的输尿管上段结石患者160例,在成功接受ESWL治疗后随机分为2组,对照组给予常规治疗;治疗组除给予常规治疗外,加用草木犀流浸液片（消脱止-M）每次4片,每天3次口服,疗程3周。结果：治疗组及对照组均行影像学检查,治疗组结石清除73例,清除率为91.25%（95%C I=85.06%-97.44%）;对照组结石清除63例,清除率为78.75%（95%C I=69.79%-87.71%）;两组清除率比较,差异有统计学意义（χ^2=4.902 0,P〈0.05）;对照组出现肾绞痛的平均次数为（1.63±1.12）,治疗组出现肾绞痛的平均次数为（0.87±0.69）,发生肾绞痛给予镇痛药治疗,二组间比较差异有统计学意义（u=5.167 4,P=0.000 0）。结论：草木犀流浸液片可以显著提高ESWL后输尿管上段结石清除率,且减少镇痛剂的使用量;其结石清除率收益为OR=0.36（95%C I=0.14-0.91）,NNT=8（95%C I=4.2-69.9）。

Clinical Observation of Therapy of Integrated Chinese and Western Medicine on Ureteral Stone in Superior Segment

Objective： To investigate the curative effect of Melilotus Extract Tablets on lithecbole after ESWL.Method： After treated with ESWL,160 cases with ureteral stone in superior segment were divided into two groups randomly.The cases in control group were treated with conventional therapy,and on the basal of conventional therapy the ones in treatment group were done with Melilotus Extract Tablets（SETUS-M）,4 pills each time and 3 times a day.Follow-up visit continues 3 weeks.Result： The cases in the two groups were all examined with imageology.In treatment group the stone was removed among 73 cases,and clearance rate was 91.25%（95%CI=85.06%-97.44%）.And in control group the stone was removed among 63 cases,and clearance rate was 78.75%（95%CI=69.79%-87.71%）.The statistical significance showed in the two groups on clearance rate（χ2=4.902 0,P0.05）.Average times of nephric colic in control group were（1.63±1.12）,and the ones of it in treatment group were（0.87±0.69）.When nephric colic attacked the patients,pain-killer was given to them.There was significant difference between the two groups（u=5.1674,P=0.0000）.Conclusion： Melilotus Extract Tablets can improve the clearance rate of ureteral stone in superior segment after ESWL,and decrease the dosage of anodyne.The stone clearance rate OR is 0.36（95%CI=0.14-0.91）,NNT=8（95%CI=4.2-69.9）
