医用几丁糖预防膝关节僵直粘连松解术后再粘连

摘    要:目的 :研究医用几丁糖预防膝关节粘连的效果。方法 :13例膝关节僵直患者行关节粘连松解术 ,随机分为几丁糖组 7例 ,术中涂抹几丁糖 1次 ,术后每周关节腔注射 3 m L,共 6~ 8周 ;对照组 6例不用几丁糖。于术后 1,2 ,3个月观察膝关节活动度以评价疗效。结果 :13例患者均获随访 ,随访时间 3~ 6个月 ,平均 4.8个月。几丁糖组各时间段膝关节活动度( 114 .3± 10 .5 ,110 .6± 13 .2 ,112 .7± 12 .8)均大于对照组 ( 94.7± 11.6,85 .3± 12 .0 ,89.6± 13 .5 ) ,经检验有显著性差异( P<0 .0 5 )。结论 :几丁糖可减轻或预防膝关节僵直松解术后再粘连的发生。

Abstract:Objective: It is to study the clinical effect of chitosan on prevention of knee adhesion after knee arthrolysis. Methods: 13 patients with knee ankylosis were performed knee arthrosis, which were divided into two groups randomly.7 patients were injected chitosan into the knee joint cavity during and after operation (3mL per week,6~8 weeks altogather), and no chitosan were used in other 6 patients as control group. The range of motion of knee was detected to evaluate the clinical effects at 1,2,3 months after operation.Results: All patients were followed up 3 to 6 months,average 4.8 months. In chitosan group,the average range of motion of knee (114.3±10.5, 110.6±13.2, 112.7±12.8) were superior to control group (94.7±11.6, 85.3±12.0, 89.6±13.5)at each period of time after operation. There was significant difference between the chitosan group and the control group (P<0.05). Conclusion: Chitosan can prevent or reduce knee adhesion after knee arthrolysis.
