局部持续冷敷在人工全膝关节置换术后患者中的应用

目的：探讨局部持续冷敷护理应用于人工全膝关节置换术后的临床效果。方法选择全膝关节置换术患者共100例，抽签随机分为观察组和对照组各50例。对照组给予常规术后护理，观察组在常规术后护理基础上增加局部持续冷敷护理，记录并比较两组患者疼痛程度、局部肿胀情况以及出血情况等。结果术后48 h对照组患者的平均疼痛评分为（7．1±2．6）分，观察组为（4．7±1．3）分；对照组平均肿胀评分为（2．2±0．6）分，观察组为（1．1±0．4）分；对照组出血量为（608．7±49．8）ml，观察组出血量（431．6±24．1） ml；观察组均优于对照组，差异有统计学意义（ t 分别为6．784，3．862，64．852；P ＜0．05）。术后观察组中1例行关节腔穿刺，抽出积液8 ml，对照组中7例行关节腔穿刺，抽出积液平均（13．7±1．9）ml。对照组平均满意度为（98．7±0．4）％，观察组平均满意度为（99．6±0．2）％，观察组满意度高于对照组，差异有统计学意义（χ2＝3．076，P＜0．05）。结论行人工全膝关节置换术后给予持续冰敷护理，可有效缓解患者所受痛苦，减轻并发症的发生情况，提高患者满意度。

Effect of local continuing cold compress on postoperative nursing of patients with total knee replacement

Objective To explore the effect of local continuing cold compress on postoperative nursing of patients with total knee replacement .Methods One hundred patients with total knee replacement were chosen and divided into the control group and the observation group , each with 50 cases.The control group received the routine postoperative nursing , and the observation group received the local continuing cold compress on the basis of routine nursing .The degree of pain , the situations of the local swelling and bleeding were recorded and compared between two groups .Results The average score of pain 48 hours after the operation was （7.1 ±2.6） in the control group, and was （4.7 ±1.3） in the observation group; the score of swelling was （2.2 ±0.6） in the control group, and was （1.1 ±0.4） in the observation group;the amount of bleeding was （608.7 ±49.8） ml in the control group, and was （431.6 ±24.1） in the observation group, and the differences were statistically significant （t=6.784, 3.862, 64.852, respectively; P〈0.05）.The case of intra-articular puncture in the observation was 1 case, and was 7 cases in the control group; the average amount of draining fluid in the observation group was 8 ml, and was （13.7 ±1.9） ml in the control group .The patient satisfaction in the observation group was （99.6 ±0.2）%, and was higher than （98.7 ±0.4） %in the control group, and the difference was statistically significant （χ2 =3.076,P〈0.05）.Conclusions The local continuing cold compress in patients with total knee replacement after the operation can effectively relieve the pain , and reduce the incidence of complication , and increase the patient satisfaction .
