Z-plate前路钢板内固定系统在陈旧性胸腰椎骨折脱位伴不全瘫治疗中的应用

摘    要:目的:探讨Z-plate前路钢板内固定系统在陈旧性胸腰椎骨折脱位伴不全瘫治疗中的应用价值。方法:对18例陈旧性胸椎骨折脱位伴不全瘫行前路开槽式扩大减压、植骨、Z-plate钢板内固定术,术后进行6-24个月(平均12个月)随访。结果:术后症状明显缓解、脊髓功能明显改善占83%(15/18),术后6个月植骨融合率达到100%,未见钢板、螺栓和螺钉松动及断裂现象,结论:应用前路开槽式扩大减压、植骨、Z-plate钢板系统内固定术治疗陈旧性胸腰椎骨折脱位伴不全瘫,具有减压彻底,术后脊柱稳定性,无需行石膏固定和植骨融合率高等优点。

Abstract:Objective:To evaluate the clinical efficacy of decompression fusion and Z plate fixation system (Z plate)in treatment of old fracture dislocation with incomplete paraplegia of thoracic or lumbar spine (OFIPTL). Methods:Eighteen patients with OFIPTL underwent anterior decompression, fusion and internal fixation with Z plate. All patients were followed up for 6 to 24 months(mean 12 months). Results:Neurological deficits were significantly improved in 15 of 18 cases postoperatively. A stable osteal fusion according to radiological criteria was achieved in all cases 6 months postoperatively. There was no implants break or loosening. Conclusion:For patients with OFIPTL, anterior decompression, fusion and internal fixation with Z plate offers the stability of thoracic or lumbar spine and a high rate of successful osteal fusion.
