35例胃肠道B细胞淋巴瘤的临床病理分析

目的:探讨胃肠道B细胞淋巴瘤的临床特点和病理诊断。方法:对35例胃肠道B细胞淋巴瘤分别进行HE染色和免疫组织化学染色。临床病理学观察内容包括患者年龄、性别、肿瘤发生部位、肿瘤的组织形态学等。免疫组织化学染色采用EliVision plus法。结果:35例胃肠道B细胞淋巴瘤的男女之比为1.3∶1;发病年龄25～83岁,平均59岁。诊断为弥漫性大B细胞淋巴瘤19例(54%);黏膜相关组织边缘区B细胞淋巴瘤(MALT淋巴瘤)7例(20%);套细胞淋巴瘤7例(20%);不能分类2例(6%)。发生于胃18例(51%);发生于小肠7例(20%);发生于结肠6例(17%);发生于回盲部、阑尾、肛管、吻合口各1例。免疫组织化学标记:所有病例均表达CD20和CD79a,不表达CD3,不同类型淋巴瘤不同程度表达CD10、bcl-6、MuM1、CyclinD1等。结论:胃肠道B细胞淋巴瘤主要分为弥漫性大B细胞淋巴瘤和小B细胞淋巴瘤两大类;胃肠道小B细胞淋巴瘤以MALT淋巴瘤和套细胞淋巴瘤多见;对胃肠道B细胞淋巴瘤中的诊断进行了分析和总结。

Gastrointestinal B-cell lymphoma:a clinical and pathologic study of 35 cases

Objective To study the clinical features and pathological diagnosis of gastrointestinal B-cell lymphomas.Method 35 cases of gastrointestinal B-cell lymphomas were reviewed.The clinical features and pathologic findings were studied.HE stains and immunohistochemical studies were performed on.Results The male-to-female ratio was 1.3:1.The age of patients ranged from 25 to 83 years,average was 59 years.Of all the 35 cases studied,19(54%) were diagnosed as diffuse large B-cell lymphoma,There were also 7 cases(20%) of MALT lymphoma,7 cases(20%) of mantle cell lymphoma and 2 cases(6%) of indefinite type.The site included stomach(18 cases,51%),small intestine(7 cases 20%),colon(6 cases 17%),and so on.The lymphoma cells in all cases were positivity for CD20 and CD79a,not for CD3.There were also various degrees of positivity for CD10,bcl-6,MuM1,CyclinD1,and so on.Conclusion Gastrointestinal B-cell lymphoma can be subdivided into DLBCL and small B-cell lymphomas.The latter group mainly concludes MALT lymphoma and mantle cell lymphoma,and so on.The diagnosis of gastrointestinal B-cell lymphoma is analyzing and concluding.
