腹腔镜下畸胎瘤剥除术中瘤体破裂相关因素分析

目的:探讨腹腔镜下卵巢畸胎瘤剥除术手术技巧,防止瘤体破裂及内容物污染盆腹腔。方法:回顾分析腹腔镜下畸胎瘤剥除术146例患者临床资料,按瘤体是否破裂分为两组,即瘤体破裂组与瘤体未破组。比较分析两组术中情况、瘤体情况及术后情况。结果:瘤体破裂组的瘤体体积及瘤体生长时间均明显大于瘤体未破裂组(P<0.01),两组年龄、术中盆腔粘连、术中出血量及术后恢复情况比较差异均无统计学意义(P>0.05)。结论:腹腔镜卵巢畸胎瘤剥除术中瘤体破裂与瘤体大小及瘤体生长时间有关,瘤体破裂后经正确处理对患者预后无明显不良影响。

Analysis of related factors of ovarian teratoma rupture during laparoscopic tumorectomy

Objective To discuss the surgical techniques about laparoscopic tumorectomy of ovarian teratoma,in order to avoid the leakage tumor contents polluting abdominal cavity and pelvic cavity.Method Restopective study of 146 patients with ovarian teratoma underwent laparoscopic tumorectomy from January 2006 to March 2010.All the patients were divided into two groups:leakage group and no leakage group,intraoperative conditions,tomor and prognosis were compared between the two groups.Results In leakage group,tumor size and tumor growth time were significantly longer than that of no leakage group(P<0.01),there was no significant difference between the two groups in the age,pelvic adhesions,blood loss and postoperative recovery(P>0.05).Conclusion Tumor laekage during laparoscopic tumorectomy is concent with tumor size and tumor growth time.After proper treatment,the prognosis of patients has no significant adverse effects.
